Thanks for posting. It helps.

Some initial thoughts - just to troubleshoot:

- Set UI quality to 1080p

- Unplug the PHL 243V7 Monitor

- Drop the refresh rate to 60

What is the model number for the LG monitor? Oddly, DXDIAG is only picking up "LG ULTRAGEAR".

    That monitor does not support 4K or 165 Hz refresh, so you need to change those settings in game to what the monitor supports.

    For what it's worth - your monitor is very close to my LG 27GL83A, which works like a champ.
