@A62047 Hmmm... I have not left for an off-lot career with my Sim with an infant or newborn - maybe not ever - but with that qualifier said, are you saying that the toddler gets cared for well by the Sims staying on the lot, but not the infant? Did you ever combine a newborn's care with and off-lot career in the past and did you find then that the adults on lot behaved differently in caring well for the newborn?
Since the infant has more needs and cannot communicate them the same, did you try hiring a nanny while you're gone to work? Or perhaps while the infant is young you could send the Sim to work on their own and stay on the lot and make sure others care for them?
I'm currently playing with a butler on my lot with 2 infants. I found that the butler did a fair amount of care for the newborns (I kept one at that stage quite a while, then had another baby on the lot as well so it's much like caring for twins.). I actually found the butler did a better job than a nanny I tried on another household. I haven't had a lot of time watching him with the infants yet, though.
