Hi there

I had this issue in April 2023. Therefore, the template version information and installation status are filled in at that time.

In my save, one of the twins between a vampire and a werewolf was a vampire and she was wearing an alien diaper. Also, I own GTW and had GTW enabled when I played this save, so the conditions are a little different than the OP.
At the time, I was testing problems with werewolf infants, so parents would always test a combination of vampires and werewolves. However, this is the only vampire infant wearing alien diapers. 

I don't think it's a problem caused by mods.

Edit:
There is a vampire infant in the save data I'm currently playing (no problems occurred while playing), so I tried using the save when she was a newborn.　(Parents are Vampire and Normal Sim)
The number of trials is 6 times. She wore alien diapers on two of those occasions. (image attached below)
After confirming the costume, I quits the game without saving, deletes all the cache files in the folder, and then performs the next test.
I was able to confirm the data during the current play, so I corrected the version information of the template and the installation status of the DLC.

Well I have seen a hat pop up in colors the game does not have. If it works then I don't think I would ever know. It only has happened twice and both times with random clothing items being placed on the sim.

Seeing as I started a new mod free save game today I guess we can use that as a point of reference.
