The game likes to kill them off. The best way is to play them until they are children. They seem safe from that state on.

Check the newspapers if there's any news about them dying. The family tree only shows the as dead if they actually show their face as a ghost which they usually don't do.

If you're not opposed to mods you can try if recover missing sims on the Overwatch mod does anything for you but you'll need to google that and use at your own risk. It's not supported by EA:
