Well this is false.. used to happen to me often due to teammates running off on their own mid-fight, only for me to rush out to save them, just for them to get knocked and killed then they rage quit, leaving me risking life and death while trying to get to them; or teammates not going into cover to get revived. There's many reasons to die, and teammates are usually 60% of the reason why.

"Teamplay makes the difference when all 3 players can take care of themselves, and are smart enough to play with a team focus. It is rarer than you think, especially in pubs."

However, you have hit the nail on the head with this perfect description.
