No reply from the TOS team
I got banned around 10 days ago and all i got was the initial email and nothing more. Only by trying to delete my account that i managed to talk to someone, it's been a week and there was no response...

I did the steps that you mentioned, but the only solution was to email an EA advisor. I'll add a screenshot of the page

And yes for a ToS appeal you would submit a ticket/email using the form there.
Sorry that this hasn't been a smooth process so far, but once that ticket is silubmitted things should start moving forward.
