Did this happen after you holstered your weapon, popped a shield cell or syringe and right after wanted to bring up the weapon to start shooting?
Because there is a stupid "bug" that will make your legend throw a punch instead of bringing up your weapon if you press your "equip weapon" and "attack" button rapidly after each other, and this bug occurs only right after you just popped a heal item some reason.
