I have a real problem with this as well. I often get killed by someone that I know I didn't see and seeing the "killer" screen I'm scratching my head thinking, "How could he possibly see me?"
It also happens some places where I can't see an enemy around a box of wall or some item, but apparently they can see me because I die.
