I was sneaking up behind someone to do a takedown on them but right when I was going to do it I got killed by the backblast of their FXM-33 AA Missile.
When, in Hazard Zone, waiting in the MV-38 Condor for extraction and everything is going well but then an enemy uses an incendiary grenade and hits the outside of the condor, which makes the flames also go through the condor and you get burned and killed before you can extract.

To me, that is just a good play. I took a break from the game because I kept dying to my gun not registering hits and dying. I literally walked up behind someone, 5 meters, with my 12M auto, unloaded about 5 shells into their head. They stood up, turned and killed me. That happened a lot frankly. Turn the corner, get the jump, they turn and kill you with their 11 damage hand gun.
I took a break for a while because of the frequency that stuff was happening. In a FPS the first thing that should work is the bullet. If you cannot get that working, your game will fail.

Constantly dying to parachute not opening even after I pressed the space bar several times!?

That's why it helps to place Irish's trophy systems near or under the Condor to stop any projectiles or have someone wait outside the Condor so that if an enemy uses an incendiary grenade or C5 and kills the rest of your squad and then tries to extract themselves you can just kill them yourself, go into the Condor and still extract succesfully.
