2042 Crashing on PC - DirectX / GPU Driver Error

2042 is freezing and crashing with the following error - please see attached to view the error. Attached is also an event that was logged from Windows Event Viewer that occurred around the same time ...

@FeatheryTrashUPDATE: Just had a hard crash while in the middle of a match of Hazard Zone, so I think it's safe to say the issue is not just isolated to Portal. I was provided with no error message this time.
