Toggle underbarrel/Fire Mode (when zoomed) keybind is broken. I used the m5a1 with grenade launcher and i cannot switch fire mode while ads(zoomed), it just switches to grenade launcher. This was working before the 4.0 update.

Hmm... By default, there is one key to switch fire mode and a separate key to switch to underbarrel. I suggest you bind to different keys, if you're not going to stick with the defaults.
On default control setting if you go to switch to under barrel launcher it mostly switches the fire mode of gun and won't switch back, while zoomed in and try to switch fire mode it will instead go to under barrel launcher.
