Cav-brawler T1 skin not unlocked

Summarize your bug I achieved the 6000 kills and assist with the CAV-BRAWLER and had the Tier 1 skin equipped before the skin was removed and locked due to a bug. With the release of update 4.0.1 I thought this issue would be resolved. Currently the Tier 1 skin is still locked and cannot be equipped even though I have completed the challenge requirements. I have attached screen shot below unable to equip the skin.
How often does the bug occur? Every time (100%)
Steps: How can we find the bug ourselves? No clue
What happens when the bug occurs? Cav-Brawler Skin is locked still after challenge requirements are completed.
What should be happening instead? I should be able to equip the skin
