How often does the bug occur? Every time (100%)

What happens when the bug occurs? First off my character is looking at the ground and won't look up even using the mouse also the buttons on my controller don't perform as mapped. ie crouch button changes weapon, ADS button crouches, Fire button changes gadget etc. All worked fine in the Beta.
What should be happening instead? It should work like it did in the beta
