I’m having same issue on PS4.
When I’m playing tonight I will try switching to my primary weapon right after first grenade leaves hand to see if it cancels out second grenade throw.
Can we get a confirmation from Dev’s that a fix is coming for this issue?????
You can stop the second grenade from being thrown if you press O on PS5 (which I have personalised to be the knife button just like in the good old days) just after you throw the first one. It will obviously cost you some time but still better than loosing the 2nd grenade for no reason 🙂

Or you can push triangle to switch directly to your primary weapon.
