This only happens in BREAKTHROUGH game mode for me and my friends. You can also see when other people are getting the bug, because they will run around without any title (name/tag). Your character might also get stuck in a crouching position and run around in that manner, until you die, which is when you cannot respawn.

The Battlefield team just tweeted out the latest details on what they plan to do in the next update. Here is what they mentioned around soldier revives:
Soldier Revives:
We're confident that we've identified the cause of the issue that was preventing you from always getting off a revive, particularly when a player is in close proximity to nearby geometry. A fix for this is on the way.
Soldier Revives:
Separately we're closing in on a possible server side fix for players who in rare circumstances find themselves unable to respawn. If that server side fix doesn't stick, we already have a further solution lined up for our upcoming update.
Yup, confirmed - still broken. On to plan 2!
after the second update, nothing really changes. still can´t revive near objects. pp29 still kills fast. and the bolte is to op and the hovercraft also. so i don´t know what ur doing.
A suicide button is the very least you should do and it should have been done long time ago, you don't have to recode the entire game for for this!
