Constant disconnects since new patch

Ever since the new patch started I have been struggling to complete a single match without disconnects. I have two completed Conquest matches out of approximately 13 matches attempted. I have no Crashdumps matching my timestamps which indicates this is a server-side issue and not on my PC. Attached is the DXdiag file.
