MY WEAPON CHANGES TO KNIFE BEFORE SHOW ME THE OPTION TO RELOAD!

Every time that my ammunition its almost done and i have to reload automatically change to knife and i have to press triangle multiple times till appear the weapon again and then reload! I can't play well! HELP! I didn't press the joystick... i made the test and happens again! I have múltiple controllers and happens all the time!
