"AN OMEN" will not unlock on XB1

So I've been trying to unlock this darn thing for two days... both on ranked private and EA servers with no lock. I've used JF video and the forum post as a guide so there no issue on my end.

ISSUE:

An Omen dog tag will not unlock despite entering correct combination on machines

other XB1 users have reported the same issue on channels such as discord 

STEPS TAKEN:

followed above guide(s) video(s)

this was performed on multiple servers (ranked)

never left server when morse code was generated

morse code decoded and entered missing letters into machines
