From the Home Screen, go to Battlefield 1, Add-Ons, and make sure that the Content has been installed correctly.
It may have not installed the additional Content that comes with the Revolution Edition.

Sorry having difficulties finding the “Add On” tab but from what I can see it doesn’t look like it re installed the Revolution DLC. How would I do that manually?

From the PS4 Home Screen, you can highlight Battlefield 1, press down with your Thumbstick until your see "Add-Ons".

Okay I found the Add Ons but it doesn’t have any of “My Add Ons” only add Ons that can be bought from the PS Store. I tried re licensing as well and it didn’t seem to work. Just to note it is a disc game, not digital download.

Also tried to deactivate and deactivate as primary and didn’t work either

That's odd, if it's a physical disk, you should have access to all the Content.
