You have indeed a Premium BF4 account. But it doesn't mean that you just get all the weapons and dog tags avilable to your solder from day1. Those are for the most part linked to certain assignments in the gameplay.
The specific Premium dog tags that you asked about are having this status right now in your soldier profile:
So here you can see that you have already unlocked 8 out of the 13 Premium dog tags in that segment. The remaining 5 dogtags are still to be unlocked by you in the game, including the one you refer to with the blue circle. All those 5 dogtags here are part of the very advanced Phantom Operative assignment in the BF4 game. Suggest you look up one of the many YouTube videos made on this, for you to see all the instructions that are required to be followed to complete that program and unlock the associated dogtags here.
