[INFO REQUEST] PS3/XBOX360 freezing/crash issues

We need some of your information in order to investigate some freezing issues with the PS3 and XBox360 console. It would be great if you guys could provide us with the following data, it will help us...

1. Had just finished a round.
2.it is freezing whenever a new map tries to load.
3. Slim
4.Was not playing. New map was loading.
5. This happens every time I try to load a new map. I have to restart my ps every time.
