multiple disc read errors during play on multiple discs - PS4

I am now on my second disc and both times after maybe 20-30 minutes of game play the game comes up with a disc read error, if I reinsert the disc it immediately says disc read error. However if I delete the game and then insert the disc I am able to play for another 20-30 minutes before the same error comes up

Okay you already tried to delete the game application in order to re-install the game. Are you playing with the PS3 disc after a next gen transfer or with the PS4 disc of the game and do you have any friends with Battlefield 4 to test if this issue is related to the disc itself?
