Origin recently had an update. Did it update the cover art for you? 

Some additional steps that can be taken:

  1. Delete Origin and download the latest version here.
  2. Clear the cache. Steps can be found here.

    Not only has it not updated the cover art, but it also displays the wrong version of Battlefield 3. (It displays Limited rather than Premium).
