When you log into the app it might be defaulting to the PC profile. Go to your account within the app and swap to your Xbox One profile.
You need to make sure you're logging into the email address you have assigned to your EA account that the Xbox Gamertag is synced to.
A way to find out what email your Xbox Gamertag is synced to is to log in to our EA Help site by using the Xbox Live login button at the bottom. Once signed in, click on Accounts at the top and then change email address. You'll see it displayed there.
