Very bad performance (cpu+gpu spikes when being shot at)

The last couple of months BF4 has been unplayable for me. The game runs great untill I am getting shot at, then the games stutters, and then I am dead. And it only happens when people shoot at me, or when some kind of big explosion goes of very close by. When I shoot at people or vehicles nothing happens, the FPS is around 70fps sometimes around 100 fps, sometimes a little lower. When this game came out, I could play this game without any problems with all the settings set to ultra, 1 screen at 1920 by 1080, I could even set the resolution scalling to 120%, I would get around 70fps it would run perfectly. I tried lowering the settings, tried out alls kinds of different settings, from ultra to low, but it still happens. I can play games like Rome Total War 2 on very high settings without this problem, so I really don't think this is hardware related. This problems began around three patches back, maybe four.

My setup:

Intel i7 2600k OC 4,4Ghz with a Mugen 2 cooler

8gb ddr3 1600

Nvidia GTX 690

Crucial M4 ssd

Windows 8.1

What I have tried to solve this problem:

I checked both GPU and CPU usage, CPU is not maxed out, around 70% , GPU is also not maxed out, SLI is on, CPU and GPU temps are both fine.

I have run the game with overclocking my CPU and GPU and without, no change.

I have tried running the game in compatibility mode for windows 7, no change.

I have tried setting the priority to high in the taskmanager, no change.

I reinstalled DX, no change.

I have disabled all background programms, no change.

I tried the latest Nvidia driver, but also a couple of older ones, no change.

I tried and repaired the game, no change.

I even reinstalled the game yesterday, and gues what, no change.

This is really getting anoying, when I first began playing this game I had almost no problems except for the problems everybody has, like the hitreg stuff. But the game ran perfect, now the last couple of months, starting as of the begining of this year, I can not play this game, what the heck is going on????? I have only one option left, and that is reinstall my windows, but I do not think this wil solve anything, all my other games run just fine. Games like Titanfall, Need for Speed Rivals, Rome Total war 2, Thief, Dark Souls 2, Castlevania los 2 etc etc run without any problems. The only game I play with a clan is Battlefield 4 and it does not work properly, I had enough of this **bleep**!!!!!

    Okay we would need to go through some basic workarounds to check if that helps or if we need to dig in deeper in your issue:

    Thanks for the reply, I did everything you said and I still have the same spikes. I even did a clean install of windows 8.1, and tried all of the things you said, and still no joy.

    I am running the following system:

    Asus 120 full hd monitor

    Intel i72600k oc 4.4ghz with a mugen 2, with 2 fans in push pull, temps are excelent.

    Corsair ddr3 1600mhz 8Gb 4x2gb sticks

    Crucial M4 SSD

    Evga Nvidia gtx 690 no overclock, and temps are also good

    I have this installed in a Coolermaster Cosmos 2, with very good airflow.

    Os Windows 8.1 clean installation fully updated

    Nvidia drivers 335.23 

    Audio realtek 889 drivers 6.0.1.7023

    Antivirus - windows security essentials

    I have tried older Nvidia drivers, and beta drivers, I have disabled all programs running in the background, I have disabled my onboard realtek soundcard, I have disables my cpu overclock, I am running high performance profile in windows energy management. I have enabled high performance, thread optimizationm, vsync of, render 1 frame ahead, single screen performance, in Nvidia controll pannel, Of course I also tried it without all these options.

    I am running the game in 1920 by 1080 120hz I also tried 60hz with vsyn enabled and disabled, I tried the lowest possible settings and the highest, with both scalling and vsync on and of, I tried a mix of low medium ultra, the GTX690 has 2gb vram, and when running Ultra it is very close to 100% full, but on lower settings I have enough vram, and I should not have these weird spikes. I also tried this with the x86 version, and I am running them as administrator.

    Again when BF4 came out I could play this game with every setting on ultra with 120% scalling on 1920 by 1080, Anti anti aliasing max with a fov of 90 and I could play the game between 60 and 100+ fps, with some fps drops, but I did not have these spikes/freezes when I am being shot at. Something changed 3 patches ago, and now I really don't enjoy this game. I can run across the map with no problems what so ever, running perfectly smooth, but when I am being shot, or a explosion goes of near me, it freezes for just a milisecond, so I cannot react fast enough to shoot back.
