I bought Battlefield 3 Premium and when prompted, entered the code in the box to unlock the EA online pass. I can download all the dlc for free, but I didn't appear to get the Multiplayer Head Start ...

Since you bought Battlefield 3 Premium, the Head Start Kit is not included. Head start kit is only available to Battlefield 3 Premium Edition. You can read more about this here: http://www.battlefield.com/battlefield3/1/premium-edition
