Checking your DxDaig you Windows 10 Pro is not up to date. Update to build 1809 this is most likely the issue.
It was working initially which suggests either an update to the game or a nvidia driver update has caused the issue.
I'll be trying to stick on 1607 if at all possible.
Why 1607? Is it because its more lightweight? if thats the case I suggest Enterprise LTSB/LTSC. it comes with no windows store/cortona. Also with LTSC you can have the latest build 1809 which should be able to run DX12 on bf5. but with LTSC i'm having the same issue here lol ☹️
