This is a known issue on EA's end, any troubleshooting on your end would only be a waste of time. EA acknowledged this problem months ago but then stopped providing updates on the matter. They know what's wrong, they know it's on their end, but they're not willing to fix it because it probably would require undoing some back-end changes they implemented, so they're sweeping the problem under the rug.
Fortunately the community did the billion-dollars-company's job and found a workaround: all you've got to do is to go in the Gameplay options and set "Player-Created Content" to "Hide" (third setting from the top of the list).
This will also hide server titles in Advanced Search, so you may want to toggle the setting when you're browsing for a game.
