Cover 3, Indiana Playbook

When running cover 3 out of the Indiana playbook (no clue if this happens in any other playbook), there are instances where the outside third safeties are aligned on opposite (or wrong) side of the field. This is out of the huddle, not even on hurry up. This allows for deep, wide open throws, leading to free touchdowns. Happened half a dozen or so times.
