Missing wide recievers out of certain formations

out of wide formations im missing a receiver. some occasions it completely changes every receivers routes to something totally random or it will give one receiver the missing receivers route in the play art and that receiver will randomly choose what route he runs. my games also been crashing constantly ever since the first update.
