After Patch DirectX Error dxgi_error_device_reset

I am not sure what is causing this because I thought I was playing yesterday (12-10-2014) without any errors.  But today every 5-10 minutes I keep getting the following DirectX Error:

DirectX function "GetDeviceRemovedReason" failed with DXGI_ERROR_DEVICE_RESET ("The device failed due to a badly formed command.  This is a run-time issue; The application should destroy and recreate the device.")  GPU:  "NVIDIA GeForce GTX 780", Driver:  34475

I bring this up because searching online about DirectX failures says that everyone is getting dxgi_error_device_HUNG, while I am getting RESET.  I don't know the significance, hence why I am making a question about it.  But pretty much the game is unplayable now.  It was not happening before and I have roughly ~30 hours on my game.  It started happening today (12-11-2014).

Any insight?

Intel i7-4770k

PNY Geforce GTX 780 XLR8 OC with 344.75 Drivers

Again, this was not happening before today, AND I get a DirectX error with _RESET instead of _HUNG (what's the significance of the difference?)

I've found that it crashes when GPU Load stays around 97%+.  I lowered my in game settings to 1080 Medium and it seems stable, however I still crash in bigger environments with a lot of textures/effects (e.g., dragon in Hinderlands).  But the crashing is very correlated with the GPU load hitting near 100%.

Shouldn't graphics cards be able to handle 100% load without crashing, or...?  Also the card is not overheating, its max temperature is around 70C.  Any help would be appreciated because I cannot tell if it is an ingame issue or potentially the graphics card.

EDIT:  Ran a Unigine Heaven Benchmark 4.0 for an hour and half and it was at 99% GPU load the entire time without a crash...  So it seems its not my card.  It really sucks that only recently has the game become unplayable and because it's origin based I cannot revert to a previous game version.
