Ea_David, is correct, in that the third slot on the potions belt is unlocked with an inquisition perk.
However, making the potion itself, can be an issue if you don't have the herbs needed to craft/refill it.
In the upper right hand corner of the potions screen, you will see the list of herb needed, and what you have in your inventory.
It will appear as red text if you do not have the required herb, for each ingredient you need to craft/refill it.
It will apear as white text, if you have enough of each herb.
But if you do not have enought of, any one of, the herbs needed, you will not be able to craft/refill it.
This is also true for when you upgrade your potions, tonics and grenades.

Community AdminIndeed. Rock Armor Tonics need Elfroot, so it should be pretty easy to stock up.
