If there ever was a quest associated with the Gates of Toth, it's long overtaken by events...I only discovered the Gates of Toth (OK, rediscovered: presumably I'd gone past them earlier) late in the game, when I was finishing off a few bits of unfinished business and noticed I had one region and one landmark unaccounted for in the Western Approach...so, when I approach the door and try to open it, it sys something like "These doors can only be opened by some kind of mechanism."
Well, OK, fair enough: so, where's the mechanism? Nowhere to be found: nor can I find any other way to approach the area from the north: and, believe me, I've spent quite along time looking, using the radar, and trying to scramble up the escarpments, all to no avail. Indeed, the very paths you would figure would take you to the right spot are closed off to you because they are the edge of the area!
Whatever quest might have gotten me there isn't in my journal: the only actual quest items I still had in the journal just before the endgame were four unfilled requisitions (including the one for cages that require Ferelden locks ?!? If anyone knows where to find those, I'd certainly be happy to learn it...).
So, sorry, your effort is much appreciated, but alas, doesn't actually solve my problem.

That first entrance near the first spawn camp when you enter the western aproach think of that as an exit gate, explore north and youll find it.

Well, OK...I've explored that area pretty thoroughly, but I'll give it another try. Do you have any specific landmarks to suggest?

So, I've now spent the better part of an hour exploring the area for the umpteenth time. What I find to be the case is:
1. You cannot actually go directly north of the entrance to the world map, as you spawn directly south of it: going north takes you up the road with the fence and thence to the world map.
2. If you continue down the hill and then turn north--past the camp--you eventually arrive at the canyons. If you continue north, you arrive outside (to the south of) the gates of Toth, as before. None of the escarpments are fully climbable, so you're limited to the canyons.
3. If you take a wider swing to the west and then turn north, you eventually arrive at the Still Ruins. Thinking perhaps that you meant that the Gates of Toth are some sort of back-exit from the Still Ruins, I re-entered them. Doing so was certainly worthwhile, as I got to discover two or thre rooms and quite a lot of loot that I'd missed the first time through. Of course it weas mostly junk, but, whatever. I did get to pick up a mosaic piece I'd missed (still only 11/12 though), along with a spec for a spirit rune that would have come in mighty handy back before the electricity dragons went extinct (swords and such were involved).
But, in all that, no connection to the Gates of Toth. If there is one, I missed it.
At this point I'm chalking it up to a bugged map, and it's far from themost annoying bug there is: I'm quite put out not to have been credited with the "Persuasive" (recruit 10 or more agents) and "Dragonslayer" achievements...the latter despite the fact that the game itself marks the quesst complete.

If I remember right you need to explore a bit then around the western part of THE WESTERN APPROACh there is an area you need to place a flag for "contruction". Youneed to go to the WAR TABLE and build it then the route to where you need to go to "get it open" will be availible. It turns out you will be taking a "back route" to get it open. Lol.

Well, indeed, and I've done all that. I may not have made it clear, but I'm doing all this right at the end of the game--indeed I am now post-endgame, so to speak, having defeated The Big C and taken my victory lap at Skyhold--so I have explored every corner, nook, and cranny of The Western Approaches, found every region and landmark except those associated witht eh Gates of Toth, fulfilled every mission I've been assigned there--indeed when I passed through the camp on my way to exploring this time, I was buttonholed by the requsitions officer and tasked with (I believe the third time) doin the geological survey, which I dutifully turned in, having a vast surplus of the materials required.
I think the quest of which you speak is glitched, in that I may have fulfilled it without going through the Gates of Totrh, and now it's too late. In any event it is of no great consequence, it just offends my completist bone, is all...
You need to find Coracavus.
That's the place that takes you to a whole new area in the Western Approach.
