Hinterlands Mosaics "The Fall"

First of all, refer to this thread on your forums for the current fan discussion of this issue:

There are, according to many (including myself, I have scoured the Hinterlands) only 10 of the 12 supposed pieces of the mosaic "The Fall", to be completed (as the game suggests) within the Hinterlands.

Can anyone provide any proof whatsoever of having either 11/12 or 12/12? If not, this seems like it may be a bug or in need of patching.

Mod edit: This issue was fixed as of patch 6. All mosaics are now completable. If you are missing pieces, make sure your game has been fully patched.

Where is the supposed "fixed" 12th piece supposed to be now? It is the only piece I am missing for "The Fall" mosaic.

I thought it was moved into the cave with the demon wolves near the horse farm.

I checked there but I may have missed it. But I have also read/seen vidios that its above the cave and I had checked there too before & after I killed the dragon in the hinterlands but I haent checked in the cave since killing the dragon.

Ok I went and checked the cave again and still no second/12th piece

Lol, this thread is like 3 years old and still no fix. Honestly, it's such a simple fix. Sad ☹️

@Rocdonald: Last I checked all twelve pieces could be found as of patch 6.

The other mosaic-related issue people were having was that certain mosaic pieces/whole mosaics wouldn't show up back at Skyhold. I'm not sure whether that ever got a fix.
