How can one improve weapons and equipment in Dragon age inquisition?

Having played most major RPG games in the past decade I have never before run into one like Dragon Age Inquisition. No instructions, no structure, basically running around killing (or trying to kill) things. Endless collection of "loot" which is basically useless. Money is also useless since not a single merchant ever has anything to buy that is better than the junk you find in the "loot," After wasting a tremendous lot of time I have level 20 characters which are still using weapons and  armor that were found back at level 6 and whenever  I try "crafting" things, neither the weapon nor anything that might improve it ever match. Ditto for armor. The chief idea of the developers seems to have been arranging the terrain so that the player cannot reach things, there are impassable mountains, dams, waterfalls, and chasms everywhere. It gets really boring running around in circles. So obviously I am missing something. Missing a great deal in fact. There is presumably some way of improving or finding better equipment as one rises in level, but so far I have not found it. Why the weapons and armor that the characters use never appear in the crafting and upgrading lists is incomprehensible to me. You give instructions to your team to hold position and instead they are under your feet regardless. You ask them to attack your target and they run off chasing after squirrels. It takes days to get the points to get a "perk," and even at level 20 there are a lot of them I have yet to reach. I have read wikis, wikias, game guides, walkthroughs, watched enough Youtube videos to make me dizzy. None of them are of the slightest help with fundamental issues. And of course the EA "Game Advisors" are of no help whatever.

So before I abandon hundreds of hours of pointless slash and burn, I hope some one in the forum can point me to some sources of information that might address the mechanics of this game so that I can actually make some progress beyond killing things and finding the same garbage over and over and over while being unable to upgrade the equipment of my character and the companions.

    I assume you know where the blacksmith is. There are crafting tables there. You can craft better equipment with the hides, cloth, and things you gather. You can also use the War Table in Skyhold to get an archanist, then with her there, you will be able to craft runes that can then be applied to equipment to improve them further. You can but recipes for crafted gear as well as potions, grenades and such at many merchants throughout the game.
