So randomly while playing my dwarf inquisitor in cut scenes now its like shes stuck smiling a full tooth grin! Her lips kinda move but its like they're pulled tight... kinda like she got botox haha. ...

I think you should probably download the Black Emporium DLC and change your character slightly using the mirror of transformation.
I tried that and It still happens...
