I know this post is old, but I just encountered the problem myself.
After an hour of doing what was suggested here with no luck, I remembered I had mods installed. So, I uninstalled them and repaired the game via Origin just to be safe and voila! Fixed. I didn't turn the mods back on while I was in there just to be safe as well.

@TaliaWilde When it comes to DLC, removing appearance mods (specifically textures, I have found hair meshes don't seem to cause problems, but hair and face textures do) is necessary. Mods which just change gameplay stuff (like abilities, loot, etc) should be fine, although some may not work in DLC areas.
Also, keeping a copy of the unmodified game patch (just save it under a different name) means you don't have to do the repair game thing, you can just copy the clean patch into your patch folder after deleting your mod folder, saves some downloading and time. 😉
