Missing Companions at Skyhold
Title says it all, really. It seems like a few of my companion (Iron Bull and Sera, especially) have disappeared, which is annoying because... well, I want to talk to them. Yes, I've been to their e...
I have manage to fix this issue but walking up to where they are supposed to be. saving directly in front of them, going out to the main menu and then going back into the game then they reappear right infront of me. Its a pain int he a** but, seems to work for now.
another "repair" if that doesnt work is to save standing in front of them, go to your hero menu> options> graphics and change your mesh Quality. I change it back and forth from Ultra to High/High to Ultra. ( http://prntscr.com/5ddbag ) and then go back to the game and they will again magically reappear standing in front of me.
again, massive pain in the arse but, seems to fix the issue for now until a patch comes out for it.

I have this problem as well, but it improved somewhat when I installed the game on an SSD. I think the characters aren't there because they haven't finished loading yet, and they tend not to spawn when you're looking at the spot where they should appear.
I suspect you're right about them loading in, I've actually seen some characters "drop" in from the sky once or twice...
Update:
I experimented a little, going to your task manager in the "processses" tab, hit the "show processes by all users" button, then find the Dragon Age Inquisition process and right clicking and setting priority to "High" helps a little with load times and characters loading in for me.
Word of warning: DO NOT set the priority to "real time". It may screw up a lot of things.
