No Romance or dialog options with Iron Bull

I can't figure out why I don't have any dialog options with Iron Bull and all the romance options stopped since the cutscene with Cassandra and Iron Bull (following "Demands of the Qun").  I've killed dragons and done a ton of side quests and companion quests yet every time I visit Iron Bull at the tavern - the dialog option is "Goodbye" no other options.  A few other posts I've looked at mention that you have to be in high approval of Iron Bull to start a romance yet that doesn't explain the lack of dialog options.  Solas outright hates my character yet I can still talk with him about the other companions and/or the main quest.  I've noticed that when I do a particular quest or make a comment I see approval ratings pop up for most of the companions yet none from Iron Bull (other than high approval for killing a dragon).

I did romance Dorian and broke it off so I could pursue Iron Bull yet I'm stuck.  I have romance options still with Dorian and can flirt with Josephine, Cassandra, Cullen (which is funny since he's not interested), Scout Harding (only outside of Skyhold) and Sera (also not interested bc she likes girls) but nothing for Iron Bull. I've also had dialog options with everyone but Iron Bull.

I haven't begun "What Pride Had Wrought" (main quest) yet as I'm having fun doing side quests and companion quests.  I'm worried that if I go further in the main quest I'll really be stuck outside of any possiblity with Iron Bull.  

Any advice or similar situations?
