Patch for ps3/360?

-I'm too having issues where I don't hear any music, background or otherwise.
-Facial details on some characters (Sera & Cullen) are sometimes there and other times gone
-Speech is messed, like the characters are looping what they're saying while speaking
-Graphics can be horrible at times, such as when reclaiming the Crossroads, the banner put up looked like the graphics were from a N64 or something.
My biggest complaint however is not being able to install the gameplay disc (disc 2). The noise only is bothersome but the game is definitely wearing on my console by not being downloaded to my hard drive with plenty of space.
