Prior to today's patch, the hitching was annoying, but the game was playable. Now, the game randomly pauses for about 3 seconds every few seconds. It does this at the title screen, while playing, in conversation, even in menus. When the pause happens, it sounds like the fan on my video card stops or slows down. I tried other games, and none are exhibiting this behavior. The game is not even playable for me, in its current state.

That's most certainly the issue for you ☹️ I'm guessing it isn't saying you need to update the game or anything? With my clean install it is telling me there's an update ready but I just pop into offline mode and it let's me play without updating. I really don't know what to suggest ☹️
I'm also experiencing atrocious performance since the patch. The night before I updated I had buttery smooth gameplay with every graphic setting maxed out. Now I stutter constantly - it is a complete slideshow. CPU spikes to 100% almost constantly. I have tried all of the troubleshooting steps listed here - cleaning out the Origin cache/temp files, clean boot. I don't have Intel Rapid Storage Technology on my computer so that isn't the culprit.
Given that I had 0 issues prior to the patch I would suggest it's something to do with the patch, not a problem with my PC. I'm honestly getting pretty disgusted with this. I have never had the experience of a mandatory patch completely killing a game before - especially when it's a (mostly) single player RPG (which I forked out $60 for and hardly got to play..). I'm getting to the point where I might have to look get a refund. Very disappointing. :/

That is my guess as well. I also have the digital deluxe, and I tried the clean install with auto-update turned off. Did not help, so I was thinking the download must have the updates built-in or they push the update right along with the main game files.

They really, really do. I was reluctant to try reinstall but then figured it surely must install base game from disk as only base info is on them. I feel for you, I really do. For everyone who is currently unable to play. It is atrocious for this to happen. Has there even been any official annoucement stating they're aware of it yet?
So this patch has claimed a fix that simply isn't true. Says it fixed the bug that causes the voice to change when you custom your Hawke. Maybe it has fixed the issue causing the bug, though some threads report it is still an issue. Yet it certainly has offered no such correction for those of us that have had the voice swap on us. Considering many like myself have shelved the game until it is fixed I, like everyone else here, would like an update as to when BioWare and EA plan to fix their broken game.

EA_David has been closing other posts regarding these issues and directing people to this thread. So they are aware of it, just not actually addressing it.

Yeah, I've been wandering threads too that's exactly what they're doing. He says to check the suggestions on this thread and if it doesn't work post your dxdiag, current games settings, and whether fullscreen windowed etc.
In addition to the previous problem I signaled, my GPU decides to play yo-yo since today :
