Points Reset Glitch and Battle Mode Glitchs to Map

I've been experiencing a problem. At random it'll take away all my skill points for each character I'm using, then it'll say I have points to spend (example: 24 points, for each character that I'm using, all three..including my character), so I'll have to reset all my skills to my characters. It's happened three times now. The first time it happened I was able to load back a few times, but the last time I wasn't able to. I just have to keep setting them in the correct slots and make sure they're starred or checked to make sure they're active. I'm not sure why this happens. Is there something I should do to make sure this doesn't happen again? I'm not sure if anyone else has experienced this problem or not. 

Another issue is a bug, at random, places while playing (sometimes even in battle mode) it'll glitch to my map and when I get out of the map, it changes to a different character. It happens more often than the skill points glitch. This one's more frustrating than skill points glitch because of all the times I'm in battle and I can't seem to make the glitch stop going to my map, making it almost impossible to fight.
Luckily, Cole, Iron Bull, and Solas finishes up the fight for me so I don't end up dying because the glitch wont let me fight.

I think its part of the same glitch, because when the map pops up while I'm fighting, when I go to another location like Skyhold, that's usually when I have to reset my points to each of the characters.

I'm using my ps3 console.

If you have any ideas why this happens? Or if they can patch this that would be great!

I also wanted to say, I love this game and all the previous Dragon Age games, I hope they make Dragon Age: 4 soon.
