I am on ps4 as well.
My first conversation with Cassandra it came to my part to choose a dialogue option. My wheel never popped up and I just stood there staring blankly at her. Eventually the conversation moved on. Minutes later I was talking to Cullen and I clicked on a dialogue option and a codex entry popped up on my screen.
I have noticed a steady increase where someone is talking and my wheel is greyed out. I hop about like a loon trying to get it to load properly until it finally does. I think this happens more if I accidently walk around when someone is talking to me.
I have had this happen on PC, the dialog wheel becomes unresponsive to mouse neither highlighting on mouse over or accepting mouse clicks, luckily keyboard shortcuts for dialog choices continue to work.
