[PS4] Lost access to room that leads to War Table and Josephine

I am also experiencing this issue.  The two problems I've fallen on during this playthrough are this one and being unable to speak with Varric's Friend on the ramparts of Skyhold.  Has a fix been found?  What was your work around?

Also, this happened to me on my PC version.
