Yeah my friend & I are having the same issue dunno what he has tried but I've tried everything I could possibly think of:
1. leaving Casandra out of the party.
2. Loading a previous save
(not fun sent me an hour back)
3. Restarting the game and running as Admin, I know that probably wouldn't make a difference but thought I'd give it a shot.
4. Leaving the instance and traveling back.
This is a bug pretty sure it needs a hot fix or a roll out patch to address the issue dunno if everyone is having it but it seems like quite a few people are.
