Similar problems on PC version.
You finish a quest in one zone, it gives you a "completed" ping, but then quest disappears from completed quests upon zoning.
Re-appears again when you go back to the zone you completed the quest in.
Unfortunately, if there is a follow up quest in a different zone - it never triggers.
Between this and hundreds of other bugs - I am suspending my DA:I play time till they fix at least half of the issues.
