I'm having the same issue. Both Cassandra and Cullen are missing. They appear as icons on the map, but simply aren't there.

I solved saving next to her and loading, just as other guy who was missing Cullen as you.
Same issue as well. The same with random NPCs. I noticed it more at the camp in Haven, and also in Redcliffe Village. Fast save & then fast reload fixes it temporarily. However like clockwork, Cassandra is ALWAYS missing whenever I fast travel into Haven. Speaking with Cullen as well fixes it.
