I'm on PS4 and I am having this exact same issue, and it has only gotten worse as I have progressed. Which just says to me that they didn't properly test it all the way through. It's unacceptable, I like this game a lot and I am unable to progress any further.

I can understand, to a degree, some of us having issues on the PC because we all have different set ups and hardware. The fact that people on consoles are having similar issues to us just smacks of half hearted QA, if it was even tested properly at all.
There is no way all the issues that are coming up would have made it through a full test.
Either it wasn't tested or they just didn't care enough. Seems like it was easier to release a buggy product a chunk of folk can't even play and try to patch it later than push it back a month or two and have it working at launch.
I know quite a few people have got refunds.
It's a shame. It is a good game but some of us have too many issues to enjoy it.
- I managed to get passed it by spamming skip before they talk. It only happened to me with the wardens in western approach in the story quest line.
