Requisition Quest Restart Bug

I've had this problem on my X360 now in around a half-dozen regions: you get the ReqOff quest, ytou complete it (sometimes right away, since often they want stuff you already have), you go to the Req Table and "craft" the "report" or whatever. Now I get a completion, and the reward (which is usually a point or two of power). So far, so good. But the next time you talk to the ReqOff, they will give you the same Req again! Presumably, if you had the patience to grind these things out, you could exploit this to accumulate disproportionate amounts of power! :-)

I first encountered this with the boat Req on the Storm Coast, but I had it also in the Mire, the Western Approaches, and thr Forbidden Oasis: I'm pretty sure I had it in the Hinterlands as well, though there it took me quite a long time to fulfill it the first time. And when I got the three successive cure quests from the doctor in Redcliffe, I was sure it was happeneing again, until I noticed it was three slightly different quests with different ingredients.
