I Have the same problem. Please Bioware, we need a fix!
I talked to him and he joined, but doesn't show up as a team member in Skyhold or on the cards when I choose my team... Does he not join in as one of the team?
No he joins as an agent not a party member

Same problem here on PC in 2017....still. Come on Bioware/EA patch this already. This bug has been in the game on all platforms since 2015.
Considering the game is over 2 years old, and it has been almost 2 years since the final DLC was released, I seriously doubt anything will ever be patched on this game that hasn't already been done so. You may see a patch if something in Windows comes out and totally breaks the game making it uplayable without a dev patch, but anything less than a total game destroying bug isn't going to be changed at this point. Yeah, it isn't ideal, but it is what it is, and pretty much any remaining bug will have to be worked around. Thankfully I haven't gotten 90% or more of the bugs I have seen posted here, but if I do, then it will be same for me, work around it or figure out why my machine decided to freak out that round....
